Personalized Member Transitions: Creating a Smoother Benefit Transition

Navigating benefit transitions is no easy task. Our personalized member transition (PMT) program makes it smoother for both plan sponsors and members. Through our high-touch outreach to members, we eliminate gaps in care, minimize member disruption, improve satisfaction and prevent unnecessary medical costs.

Did you know?

  • 20% of prescriptions are never filled
  • 50% of prescriptions are taken incorrectly
  • 28% of patients fail to refill a prescription on time
  • 22% of patients take a lower dosage than prescribed

Become part of the solution to healthy outcomes through our seamless and positive member transition experience.

Why PMT Matters

The Navitus PMT program offers high-touch onboarding support, giving both plan sponsors and their members a smooth benefit transition. Members can expect our highly-skilled and friendly clinicians to proactively communicate with them and their medical providers to help simplify and streamline prescription transitions. They provide the personalized care and support that your members deserve. Our PMT program provides:

  • Value to benefit managers through our comprehensive and proactive approach to member-related questions
  • A positive member transition experience that helps save members and plans money by switching to a lower-cost, clinically effective product
  • Improved member satisfaction
  • Enhanced speed to therapy

How PMT Works

Our PMT program follows a four-step process to give your members piece of mind through proven solutions that help them stay on top of their health.

  1. Member Identification: A clinical transitional analysis identifies members on maintenance medications whose pharmacy benefits are changing.
  2. Member Mailing: Identified members receive a clinical transition letter that outlines any impact on them and provides them options.
  3. Member Outreach: A Navitus clinician proactively reaches out to discuss the benefit change, and ensure that members don’t go without therapy. They combine a human touch with clinical expertise to educate members, answer questions and discuss solutions.
  4. Provider Outreach: Our Navitus clinicians don’t stop there. They complete the transitional care loop by also contacting the member’s provider to notify them of the benefit change and provide resources to facilitate any needed adjustments to medication therapies. If applicable, they will facilitate the request for a new prescription to be sent to the member’s pharmacy.

Core to our vision at Navitus, our PMT program enables us to help improve health outcomes for the people we serve.
