High-Touch Approach
How it works
Our PMT program follows a four-step process:
Member Identification: A clinical transition analysis identifies members on maintenance medications whose pharmacy benefits are changing.
Member Mailing: Identified members receive a clinical transition letter that outlines any impact on them and provides them options.
Member Outreach: A Navitus clinician proactively reaches out to discuss the benefit change, and ensures that members don’t go without therapy. They combine a human touch with clinical expertise to educate members, answer questions and discuss solutions.
Provider Outreach: Our clinicians don’t stop there. They complete the transitional care loop by also contacting the member’s providers to notify them of the benefit change and provide resources to facilitate any needed adjustments to medication therapies. If applicable, they will facilitate the request for a new prescription to be sent to the member’s pharmacy.
