Improve and Maintain Star Ratings

RISE is a comprehensive Star Ratings Improvement program that focuses on positive outcomes for Medicare Part D (Part D) clinical measures, including:

  • Medication adherence for diabetes medications
  • Medication adherence for hypertension (RAS antognists)
  • Medication adherence for cholesterol (SUPD)
  • Polypharmacy: Use of multiple anticholinergic medications in older adults (Poly-ACH)
  • Concurrent use of opioids and benzodiazepines
4 to 6 percentage point increase in each adherence measure. Performance increase in 2023 versus 2022 for a DSNP client.

Combined, these measures comprise a large proportion of the Part D Star Ratings calculation and heavily influence the largest weighted measure: Drug Plan Quality Improvement. When paired with our Medication Therapy Management (MTM) program, health plans have the opportunity to provide comprehensive clinical support to their members to improve outcomes, reduce cost and improve Star Ratings.

Our Proprietary Application

RISE’s proprietary, web-based application performs three core functions: automated daily qualification, clinical intervention and on demand reporting. It is easy to use and its customizable settlings allow health plans to delegate interventions to the Navitus Clinical Engagement Center, complete interventions on their own using out software or use a hybrid access model.

The on-demand reporting feature includes near real-time data for unparallelled insight into plan goal progress. The analytics identify interventions to target members most in need of support. Our demonstrated results prove that interventions completed maximize Star Ratings impact.
