How PMT Works

Our PMT program follows a four-step process to give your members piece of mind through proven solutions that help them stay on top of their health.

  1. Member Identification: A clinical transitional analysis identifies members on maintenance medications whose pharmacy benefits are changing.
  2. Member Mailing: Identified members receive a clinical transition letter that outlines any impact on them and provides them options.
  3. Member Outreach: A Navitus clinician proactively reaches out to discuss the benefit change, and ensure that members don’t go without therapy. They combine a human touch with clinical expertise to educate members, answer questions and discuss solutions.
  4. Provider Outreach: Our Navitus clinicians don’t stop there. They complete the transitional care loop by also contacting the member’s provider to notify them of the benefit change and provide resources to facilitate any needed adjustments to medication therapies. If applicable, they will facilitate the request for a new prescription to be sent to the member’s pharmacy.
